My guess at an actual explanation is that somebody in the herbs and spices supply chain between here and another hemisphere has been stealing product and topping up the shipments with whatever local weeds look approximately the same when ground up; and one day they added some wolfsbane to the mix.

OK, so York Region public health is telling people to throw out two "Mr. Right" brand products: https://www.cbc.ca/news/canada/toronto/mr-right-brand-aconite-poisoning-york-restaurant-1.6569196

One is a spice mix, and one is a Chinese-medicine mix which contains monkshood, but it's supposed to have been processed to reduce the toxicity. So it's looking more like two dangerous errors at the factory -- someone didn't prepare the monkshood properly, and someone allowed one product to contaminate another.
