OK, so York Region public health is telling people to throw out two "Mr. Right" brand products: https://www.cbc.ca/news/canada/toronto/mr-right-brand-aconite-poisoning-york-restaurant-1.6569196

One is a spice mix, and one is a Chinese-medicine mix which contains monkshood, but it's supposed to have been processed to reduce the toxicity. So it's looking more like two dangerous errors at the factory -- someone didn't prepare the monkshood properly, and someone allowed one product to contaminate another.
